Catching signs of #cancer early is crucially important to the disease management and survival rates, so the question is: how can we find out if there is something wrong going on early enough?

The first generation of high-throughput technologies was predicated on finding mutated (tumor) DNA in the individual's body (somatic), different from their (normal) DNA. Tumor/Normal (T/N) comparisons of the individual's samples, biopsies or ctDNA will indicate if there

are signs of cancer already underway. So why do we need #epigenomic profiling if we can do all this with the detection of mutations? Well, the argument is that if you can see sufficient mutated copies of DNA, cancer may already have progressed beyond the initial stages.

Then people asked the question: is there an earlier mark that we can detect for earlier stages of cancer before the Tumor/Normal mutations signal amps up? Here is where #epigenomics profiling took hold.

This may have called for a lower “per lakh” premium, but it does not justify a discount of 37% for a 400% higher cover

Something you need to understand well.

Say you have a mobile plan with a quota of 3GB of data a day.

The average usage is say 2 GB a day.

Now if you apply for an upgrade to 10GB a day - the telco knowing this will rarely be utilised, can upgrade the plan at an extremely low cost, and not at the per GB rate.

Similarly, when you today buy a cover of 25 Lakhs. Since the average claims are say 3 Lakhs today, there are very tiny chances you will use the cover over 10 Lakhs in the short term.

So whatever amount the insurer charges today over 10 Lakhs is a bonus.

A senior doctor at one of the biggest London hospitals tells me that all elective surgery has now been cancelled and that roughly 80-85% of beds are currently being used for COVID patients.

At one hospital in London a specific team has been set purely to aid with the patient discharge process. Why? Simply to free up as much bed space as possible because demand is higher than it has ever been.

[THREAD] I want to talk a little bit about how people who are struggling with their mental health may appear to the outside world and why some things contribute to stigma even when we are not aware of them. I often complaints from families or partners.

They would say this person no longer listens, is absent, always on their own and may take it personally. Let’s go through why that happens and what explanations we have for these shifts. First of all, social withdrawal is one of the main symptoms of many forms of psych issues.

Social situations (yes that includes family and loved ones) takes a lot of energy. We don’t notice that for the most part when we are doing well but when we start struggling, every word, listening to conversations, etc...takes so much energy and can be hard to maintain.

My last #tweetorial focused on the GCS. Now let's look at another hugely important clinical window into the brain: the pupil. What does a 'blown pupil' really imply? What about small fixed pupils? To understand this, we need to see how pupil size is governed. #FOAMed 1/20

The pupil has two functions, constriction and dilation, both under autonomic control. Sympathetics dilate the pupil and open the eyelid - easy to remember, because in 'fight or flight' mode, maximum information needs to get in to keep you alive. 2/20

By contrast, constriction is parasympathetic. Its main function is to regulate the amount of light on the retina, so in response to high light intensity (like a pen torch), both pupils constrict. 3/20

These parasympathetics have a straightforward course (literally). They hitchhike on the third nerve, which runs forward from the midbrain to the orbit to supply most of the extraocular muscles and eyelid. As they're superficial, they're vulnerable to extrinsic compression. 4/20

Plenty of things can compress the third nerve and produce mydriasis (a dilated pupil) - most classically a posterior communicating artery aneurysm. But in the context of an acutely comatose patient, it suggests something worrying: 'uncal herniation'. Why is this? 5/20

can we all have a touch grass moment for a second. can we please remember that idols are real people. real people with problems they might not have the access to solutions for. many of them young, early 20’s, brains still unfinished.

yes, there is a level of responsibility when it comes to what you share with your audience, especially if they are young and impressionable, however, idols are not going to be hyperonline like most of us, and are not going to know the ins and outs of what might be acceptable +

+ especially in regards to something as nuanced and varied from place to place as diet culture. no, i don’t think talking about going on a diet as an idol is inherently bad. they are sharing their life with us.

as it stands, their jobs revolve around being open with us, and if it provides some comfort to them to be able to share things like this, then i don’t think it should necessarily be shamed as hard as it was this morning.

i saw a tweet saying to pick up a book on fat politics to see why this whole debacle is just unbridled fatphobia from the artist.
