I have now read the study, and I teach critical appraisal as part of my job, so let's dive into it a bit.

[THREAD] I want to talk a little bit about how people who are struggling with their mental health may appear to the outside world and why some things contribute to stigma even when we are not aware of them. I often complaints from families or partners.

They would say this person no longer listens, is absent, always on their own and may take it personally. Let’s go through why that happens and what explanations we have for these shifts. First of all, social withdrawal is one of the main symptoms of many forms of psych issues.

Social situations (yes that includes family and loved ones) takes a lot of energy. We don’t notice that for the most part when we are doing well but when we start struggling, every word, listening to conversations, etc...takes so much energy and can be hard to maintain.

This is why they may get irritated and lash out. It is because the energy is gone and they can no longer keep up the mask of normalcy. It is important to respect their space and boundaries while they are going through this hard time. Don’t force them to do things they don’t want.

Also, if you talk to someone who is struggling with their mental health, they may seem absent or like they are not caring at all. Well, from the perspective of the person who is struggling, their concentration is on their thoughts, their bodies, checking for panic attacks.

I struggled with intense acne for 3 long years in high school.

The arc of the pimple commercials was always the same:

1. Guy with pimples hides from his crush

2. Someone tells him he's ugly

3. There's a party

4. Someone recommends some skincare product

5. Guy uses product, goes to party, kisses the girl

The message this reinforced to people for whom the product didn't work (aka me) was also always the same:

As long as you have acne, the other sex won't be interested in you, so don't bother.

It's a pattern deeply embedded in Western culture:

Find a flaw, worry about it, try a quick fix. If it doesn't work, go back to worrying.

Repeat this cycle until you find an even bigger inadequacy.

This may lead to some improvement, but it inevitably leads to self-loathing.

Now, let me present to you: Japanese skincare ads. They don't target teenagers.

Pimples aren't a flaw to be overcome, just a part of life.

"If you take care of your skin, acne might happen but won't ruin your day."

It's been 13 months since my craniocervical fusion surgery and 7 months since my tethered cord surgery. My diagnoses were craniocervical instability, tethered cord, ME/CFS, MCAS, POTS, and mold illness (which may or may be fully explained by MCAS). 1/n

Before my first surgery, I couldn't sleep inside a house b/c of mold illness/MCAS. I got paralyzed, unable to speak, and barely able to breathe, sometimes 8 or more times a day. This could result from tilting my head back, speaking too much, a loud sound, a tap on the head. 2/n

Traction (pulling my head away from my body) brought me back from these episodes. I couldn't ever be left alone. I wasn't bedbound, but I could do essentially nothing for myself. 3/n

By that point, my symptoms were extreme even for CCI, and they were quite unusual for ME/CFS. But I started out with classic ME/CFS -- PEM, POTS, unrefreshing sleep, cognitive impairment, low NK-cell function, wacky cytokines, sky-high IGGs for a bunch of viruses. 4/n

The last year has been HARD. These surgeries are brutal and hard to recover from. Needing a second surgery when I hadn't yet fully recovered from the first was emotionally and physically rough. 5/n

3) This is based on the erroneous assumption that lockdown causes economic damage - the truth is, covid causes economic damage, and countries that don't deal with covid get damaged the most (Britain has the worst death toll and the worst economic contraction in Europe currently)

4) the first paragraph of this contains some very skewed stats: most scientists agree that IFR is closer to 1% than 0.23%, and 60% vaccination/infection is necessary for herd immunity, not 30-40%

5) The Stanford study is a mess - it takes data from only 10 countries (not enough to draw any meaningful comparisons), and ignores those like Brazil where the result of non-lockdown policies has been catastrophic....

5)... Sweden has horrendous death rates compared to other Nordics (unacknowledged), and the study fails to consider many of the additional extreme measures taken in S Korea (testing to a degree not undertaken anywhere else in the world) that made lockdown unnecessary there.

Handoffs come in many flavors in medicine. In general, the highest risk handoffs are when the patient is really sick and the handoff is permanent and not temporary. So in some ways, yes has elements of a risky handoff. America is definitely sick and the handoff is permanent.

Ideal handoffs are a transfer of content and a transfer of professional responsibility. The goal of content transfer: to achieve a shared mental model or shared vision of the patient. Professional responsibility usu= does receiver accept? Now it’s been will sender relinquish?

While both parts key, the transfer of professional responsibility is a must. The transfer of content also rests on the quality and accuracy of information transferred, the sender investment to transfer the information, and the receivers ability to understand & act on it.

In this case, there is concern no or sparse content is transferred. Even with a checklist, there may reason to question the content. In fact, too much dependency on content during any handoff could actually harm decision making of the person taking over due to an anchoring bias.
