1/ The classic presentation at morning report for hypercalcemia starts with polyuria, constipation and confusion. #UncleBob wanted to understand why - stimulated by @CuriousClinPod ? @HannahRAbrams @tony_breu @AvrahamCooperMD

 2/ Let's start with confusion. Finding information on this is very non-specific but I think this quote helps: High calcium levels can be a catalyst for neuronal demise, possibly due to glutaminergic excitotoxicity and dopaminergic and serotonergic dysfunction.
3/ But colleagues and learners know that I am most interested in the polyuria. I have taught that hypercalcemia can cause nephrogenic diabetes insipidus, but the mechanism was unclear. Let's review how ADH works and then look at an interesting study that suggests an answer.
4/ Antidiuretic hormone stimulates water reabsorption: stimulating insertion of "water channels" or aquaporins into kidney tubule membranes which transport solute-free H20 through tubular cells & back into blood, -> decrease in plasma osms and an increase urine osms.
5/ Thus we need to stimulate ADH production and have aquaporins (specifically aquaporin-2) available for transport. In general nephrogenic DI occurs because of the lack of aquaporin-2(AQP2).

7/ So now we have to understand autophagy! From the article: Autophagy is a highly conserved biological process that involves removal of protein aggregates and damaged organelles and the subsequent transport of these components to the lysosome for degradation
8/ Hypercalcemia causes protein degradation. This occurs for many proteins, and specifically AQP2. Degradation occurs very rapidly in animal models. Thus, patients will have dilute urine because ADH cannot interact with the missing AQP2.
9/ As an aside, this likely also occurs with hypokalemia (but not as dramatically). The animal studies also show that once the hypercalcemia resolves, AQP2 quickly returns to normal levels.
10/ The constipation likely occurs secondary to the volume contraction from the polyuria. This can be severe enough to elevate to obstipation.
11/ Lowering the calcium level to normal resolves these symptoms. I hope this discussion of mechanisms makes sense.

- 100 of them are sick
- 999,900 of them are healthy

After therapy sessions, it is occurring to me how my parents and maybe most desi parents don't really grow as parents. For example, a person would be a very different to a baby and to a teenager because the needs and vulnerabilities are so different.

But here, I feel that growth for parents stagnants sometime around when a child is 12-13. None of our parents really talk to us about puberty and growing up. None of us are taught or can talk about relationships because of moral reasons. So what do you have?

You have parents who are still parenting their adult children like kids, not apologising to us but asking us to eat, reminding us to wear jackets, locking up cars. They care. But they are coming from a care where the parent is always concerned about keeping the kid safe and alive

which imo really reduces the kind of parental advice and care one needs as an adult. Because adults know enough about the world to no longer be infantilized but we still have parents who expect us to be the same children because they're still the same kind of parents.

This is why so many of our parental relationships are strained or just based on civilities. We're forced to hide a lot from them because parents refuse to grow as parents and acknowledge us as adults. How many of us are still lectured, have curfews, other restrictions?

1/23 We haven't had a lot to celebrate during this pandemic. But achieving the milestone of offering vaccine to the over 70s, health and care workers and the most clinically vulnerable just before target date is a huge cause for celebration, given the scale of the achievement.

2/23 When Margaret Keenan got her first jab in Coventry on December 8th last year, who’d have thought that, by Valentine’s Day, we’d have vaccinated over fourteen million people across the UK in just ten short weeks? Amazing, extraordinary and truly, really, world class!

3/23 I’ve been struck by how much joy and relief has been brought to those doing the vaccinations and those receiving them. A GP told me yesterday that nothing had given him greater pleasure in his long career. And that the patient's relief was sometimes overwhelming.

4/23 It's vital to recognise the extraordinary contribution of the staff and volunteers who have made this happen. One of my close family had their jab yesterday and was in awe of the volunteer standing outside in the freezing cold for a four hour shift meeting and greeting.

5/23 What I wanted to share in this thread was the slightly untold story of some of the extraordinary challenges the NHS has had to overcome to deliver this target. Requiring superhuman effort from NHS leaders and staff, way beyond the call of duty. Five challenges set out below.

@sbarnettARK Epigenomic biomarkers are becoming more established for #LiquidBiopsy and #CancerScreening, and we have seen the big players positioning themselves in this #epigenomics race recently.

Catching signs of #cancer early is crucially important to the disease management and survival rates, so the question is: how can we find out if there is something wrong going on early enough?

The first generation of high-throughput technologies was predicated on finding mutated (tumor) DNA in the individual's body (somatic), different from their (normal) DNA. Tumor/Normal (T/N) comparisons of the individual's samples, biopsies or ctDNA will indicate if there

are signs of cancer already underway. So why do we need #epigenomic profiling if we can do all this with the detection of mutations? Well, the argument is that if you can see sufficient mutated copies of DNA, cancer may already have progressed beyond the initial stages.

Then people asked the question: is there an earlier mark that we can detect for earlier stages of cancer before the Tumor/Normal mutations signal amps up? Here is where #epigenomics profiling took hold.

I think @SamAdlerBell in his quest to be the contrarian on Fauci gets several things wrong here. 1/


First, the failure last year actually was driven by the White House, the #Trump inner circle. Watch what's happening now, the US' scientific and public health infrastructure is creaking back to life. 2/

I think Sam underestimates the decimation of many of our health agencies over the past four years and the establishment of ideological control over them during the pandemic. 3/

I also am puzzled why Tony gets the blame for not speaking up, etc. Robert Redfield, Brett Giroir, Deb Birx, Jerome Adams, Alex Azar all could have done the same. 4/

Several of these people Bob Redfield, Brett Giroir, Alex Azar were led by craven ambition, Jerome Adams by cowardice, but I do think Deb Birx and Tony tried as institutionalists, insiders to make a difference. 5/

Valakhilyas are 60000 Rishis who are thumb-sized. They were born to Kratu and Kriya. Rig Veda says that they sprang from the hairs of Brahma.
They move in front of Surya dev’s chariot as Sun moves daily and
shine like brilliant lights due to severe tapasya.


Tamil literature says that the main task of Valakhilyas is to prevent human beings from being scorched. They absorb the excess heat from the sun by travelling in front of him. They, in turn, get energy from Skanda and Ma Durga. The food of Valakhilyas is only wind.

Once, Rishi Kashyap performed a yagna and invited all the Devas and Rishis to help him. Indra brought a whole mountain of trees for firewood. The Valakhilyas were hardly able to lift anything but leaves. Even when they were moving leaves, they fell into rain water puddles.


Indra made fun of them. So they cursed Indra saying that his slayer would be born as the son of Kashyapa. When Indra apologized, they modified the curse that the son shall be initially an enemy of Indra, but later will become his friend.


This son born later to Kashyapa was Garuda. He flew to Indraloka to get Amrita and defeated Indra.
Later, he restored amrit vessel to Indra and was honoured as the King of birds and as the mount of Sri Vishnu.

Ancient but beautiful temple dedicated to Hanuman. This temple is a legendary one. According to the Sthala Puranam of Sanjeevi Rayar temple,it is associated with an episode from Ramayana.


Legend story -Hanuman was flying from Himalayas to Lanka bearing Sanjeevi hill to revive Lakshmana during battle between Rama& Ravana, a piece of the Sanjeevi Parvat fell at this place.Many believe this legendary story & even today that no one here dies of any kind of poisoning.

From historical background of the temple it is known that temple was originally built by a great scholar &administrator Kotikanyadanam Lakshmikumara Tatacharya. Temple itself&village were named following name of great scholar Tatacharya, who was then popularly known as Ayyangar.

He was the royal preceptor of Vijayanagara emperor Venkata Raya who had his capital at Chandragiri in AP.Upon returning from a pilgrimage,robbers tried to attack Tatacharya at this place. Being a great devotee of Hanuman,he instantly composed a poem in Sanskrit praising Hanuman.


The bandits immediately ran away &Tatacharya decided to build a temple dedicated to Anjaneya at that very place.This Sanskrit poem is called Hanumad Vimshati, has been inscribed in Grantha script inside this temple.The huge temple tank called Tatasamudram, in honor of Tatacharya.
